Not Getting Results on GLP-1 Medications? Your Gut Could Be Sabotaging Your Efforts!!
Last month I attended a full-day seminar on GLP-1 medications and weight loss, and I was so interested to learn that there is a connection between your gut health and the effectiveness of these medications.
GLP-1 receptor agonists have changed the landscape for weight loss and metabolic health. For many, they offer significant improvements in appetite regulation, blood sugar control, and weight management. But here’s the part that doesn’t get talked about enough in mainstream conversations:
Not everyone responds the same way though. Why?
Some people see incredible results, while others feel like they’re doing everything “right”, spending lots of money and still not getting the outcomes they expected and end up feeling disheartened. Emerging research is now pointing to a powerful—and often overlooked—factor in this difference in people’s results:

The Gut–GLP-1 Connection
GLP-1 (glucagon-like peptide-1) is not just a medication you swallow or inject—it’s a hormone naturally produced in the gut, you make it every day, and it plays a key role in:
Slowing gastric emptying, so you feel fuller for longer
Regulating appetite and satiety, meaning less snacking
Supporting insulin secretion, which helps regulate your blood glucose levels
Influencing inflammation and metabolic health
So, it makes sense that the environment of the gut itself—particularly the microbiome—could influence how well GLP-1 medications work. I can test your microbiome and the level of inflammation in your gut, which can give us useful information on how to support you correctly.
What the Research Shows
Studies comparing people who had great results with GLP-1 receptor agonists versus those who did not, have identified clear differences in gut microbiome composition and function.
Key findings include:
Higher levels of Bacteroides species in those who respond well to treatment
More pro-inflammatory microbial profiles in non-responders
Reduced effectiveness of these medications in antibiotic-treated mice, suggesting gut microbes are essential for the medicine to work well
Evidence that gut microbes actively contribute to the metabolic effects of GLP-1 medications
This tells us something important:
GLP-1 medications don’t work in isolation—they work in partnership with your gut.
Dysbiosis: Silently Sabotaging Your Efforts
If your gut microbiome is out of balance (we call this dysbiosis), several mechanisms may interfere with your response to GLP-1 medications:
1. Impaired Hormone Signalling
A disrupted microbiome can alter the production and signalling of gut hormones, including GLP-1 itself.
2. Increased Inflammation
Pro-inflammatory microbial patterns can reduce metabolic responses and interfere with insulin sensitivity.
3. Gut Barrier Dysfunction
When the gut lining is compromised, immune activation increases, moving the body away from optimal metabolic function, which affects weight loss.
4. Altered Nutrient Processing
Gut microbes influence how we extract and utilise energy from food, which may impact weight loss results.
Why This Matters Clinically
If someone isn’t responding to GLP-1 therapy, it may not be a “compliance issue” or a failure of the medication.
It could be that:
The gut environment isn’t supporting the effectiveness of the medication
There is underlying inflammation or overgrowth (e.g., SIBO, dysbiosis)
Digestive function is impaired
This is where a root-cause approach becomes essential.
How to Improve Your Response Before Starting GLP-1 Therapy
One of the most empowering things about this research is that it opens the door to optimising outcomes before you even begin treatment.
Here’s where I would focus clinically:
Accessing overall health and nutrient status, as this is essential for a healthy gut, especially when nutrients can become depleted while undergoing treatment.
Access gut health and function, through my intake forms and if possible with comprehensive microbiome stool testing to gather clinical information on microbiome diversity, richness, the levels of systemic and gut inflammation, so we can optimise these.
Create a plan for nutrient-rich, easily digestible small meals that will give you the best support, when appetite decreases with this medication.
Natural medicine also offers many tools to address and minimise the side effects that can occur with GLP-1 antagonist medications, including nausea, reflux, diarrhoea, constipation, fatigue and headaches.
