·         in a zoom consultation or in clinic consultation

·         when you send me information in an email

The purpose of collecting your personal information is to access your current health situation and past medical history. Having all of your health information allows me to be able to assist you to reach your health goals. Understanding all the factors that play a role in your health allow me to make informed decisions about what supplements and dietary & lifestyle changes will give you the best results in a safe and ethical way.

The types of information I need to collect during a consult will include

·         Medications

·         Test results

·         Full medical history

·         Social and family history

·         Past treatments

I am committed to securely storing and handling your sensitive information.

·         Sensitive information is stored in a filing cabinet in a secure office and on a password protected computer that has up to date online security.

·         Information is accessed by me and only contact details are shared with our reception staff.

·         Occasionally on a needs basis, reception staff need to access other information like supplements I have prescribed to you or test results.

·         If you would like to access your information or update something, please email me at tummyrescue@gmail.com and I will respond within 24-48 hours. If your request will take more time, I will provide a timeframe to you.

·         I do not store sensitive information online or in the cloud.

Sensitive information may be collected from children under the age of 18 under the following circumstances:

·         in the presence of their parents/guardian’s

·         with their parent or guardian’s full consent

All information collected from minors is securely stored in accordance with this privacy policy.

All archived sensitive information is securely destroyed after 7 years.

You may choose not to provide me with your sensitive information. However, you do not provide me with complete and accurate information about yourself and your health, I cannot safely and effectively help you with your health concerns.

Disclosure of Information

Reasons why I may disclose your information include:
